Name
[Required] This needs to be your name, or if you are ordering for a company, the name of a contact for your company who can authorise purchases and has some authority to make decisions.
E-mail address
[Required] This is the email address of the person whose name is given above.
Company name
This is the name of the company for which this purchase is being made (leave it blank if the purchase is for a personal system). If applicable, and it fits, please include the division or region, whichever is appropriate.
Address
[Required] This is the physical mailling address for the person or company making the purchase. There are two lines provided for this (this should be enough, but if it takes more, please try to combine lines).
City, State, Zip
[Required] This is the city and state (or province) and postal code section of the physical mailling address for the person or company making the purchase. Please put the city and state/province and postal code in this one blank (format it as you would for an address in your country).
Country
[Required] This is the country part of the physical mailling address of the person or company making the purchase.
Home telephone
[Required] This is the home phone number of the person making the purchase, or the direct phone number for the representative named above (in the Name and E-mail sections). Please include extensions if applicable.
Work telephone
This is the work phone number for the person making the purchase (if applicable), or the main telephone number for the business making the purchase. Please include any extension if applicable.
User name
[Required] This is the name that the program will display as the entity to whom it is registered. This should be the user name for personal purchases, or the company or division for corporate purchases.
Other options
If other services or options are available for the product, you can select here to be included in them, or perhaps which of them you want to be included in if there are several options or services.

In order to return to the PayPal registration form, you will have to use your browser's `previous document' function. This is usually found as a button labelled `Back' somewhere in the browser display. If you were to follow a link back from here, it would clear your form.
